Family Support
The Family Support Program enables individuals who are disabled to live successfully in the least restrictive appropriate community environment. The family support program provides training and support education, money management, medical assistance, transportation and utilization of community resources, (examples: Social Security Office, Public Aid Office, etc.). Referrals are made as necessary. Services available with the family support unit:

·  CILA

·  Case Coordination

·  Respite

·  Supported Living Services

·  Medicaid Waiver

Why use Family Support?

The Family Support Program is designed to help families carry out a day-to-day service that they may be unable to do that day.  When an individual who is developmentally disabled is cared for by the family, many responsibilities have to be met.  Family Support can help with these responsibilities if it is possible to do so.

The following services and more are available if it will help the individual remain in the least restrictive environment.

Dentist Appointments

Doctor Appointments

Transportation

Calling an appropriate service for you

Linkage to community agencies

Be an advocate
